To initiate wellness care for a new animal resident, please complete the form below. Submitting the form will trigger the following sequence of care:

1. We will authorize a wellness exam and basic preventative care at the Peace of Mind Dog Rescue Harry and Jaynne Boand Clinic in Monterey.

2. Animals who have received their wellness exam and preventative care will be able to be spayed or neutered at the Monterey SPCA. Please contact the SPCA at the Shelter Manager's direct number at (831) 264-5441 to schedule this appointment, and let the Shelter Manager know that the pet is a SHARE pet. Do not schedule the spay/neuter appointment until after the initial wellness exam and vaccinations have been completed.

Note: Due to the high demand for services at the above-mentioned clinics, appointments must be kept, and there will be no opportunity to reschedule missed appointments.
