1. Does the animal have an immediately life-threatening or life-compromising injury or illness that requires urgent care?

Your answer yes, is a prerequisite to qualify for assistance from BirchBark Foundation. Please continue to the next question.

No
No, the animal does not have an immediately life-threatening or life-compromising injury or illness. BirchBark Foundation financial assistance grants are only for use in urgent situations that require immediate treatment, and cannot be used for basic or routine care. Due to limited funds, we are also unable to help with costs toward treatment for ongoing conditions.

2. Has the animal recently been seen by an ASSOCIATE MEMBER veterinarian regarding this issue?

Your answer yes, is a prerequisite to qualify for assistance from BirchBark Foundation. Please continue to the next question.

No
No, the animal has not recently been seen by a veterinarian. You must have a tentative diagnosis and recommended treatment plan in place from an ASSOCIATE MEMBER HOSPITAL to apply for a BirchBark Foundation financial assistance grant. Once there is a recommended treatment plan in place, please comeback to our website to review our financial assistance grant guidelines to determine whether your situation is eligible for funding.

3. Has the ASSOCIATE MEMBER veterinarian finished the diagnostic work (x-rays, blood work, etc.) and given advice on what treatment is needed?

No
No, the veterinarian is recommending more diagnostic work. Unfortunately, our financial assistance grant program is generally unable to assist with the cost of diagnostic work such as office visits, x-rays or testing. Once all diagnostic work is done and there is a recommended treatment plan in place with an ASSOCIATE MEMBER, please come back to our website and review the our financial assistance grant guidelines to determine whether your situation is eligible for funding.

4. According to the ASSOCIATE MEMBER veterinarian, is there a good prognosis and can treatment be mostly completed in one visit or hospital stay?

Yes
Your answer yes, is a prerequisite to qualify for assistance from BirchBark Foundation. Please continue to the next question.

No
No, the prognosis is poor and/or the animal will need ongoing care. Due to limited funding, the BirchBark Foundation financial assistance grant program is unable to help with the cost of care for animals with a poor prognosis, or with costs for ongoing treatments or ongoing conditions. Please visit our resources page for a directory of other financial assistance programs.

5. Has the applicant applied for Care Credit?

Your answer yes, is a prerequisite to qualify for assistance from BirchBark Foundation. You will have to submit documentation of approval or rejection for your application to be considered complete. Please continue to the next question.

No, the applicant has not applied for Care Credit. Please note that applicants are required to seek other sources of funding before applying (including savings, 401k, family and friends). Before BirchBark Foundation financial assistance grants can be considered as an option, pet owners must first apply for Care Credit. Please visit the Care Credit website to submit an application. Applications can be submitted individually or with a co-signer.

6. Is the applicant’s total household income within 250% of the federal poverty guidelines (see chart)?

If the applicant's yearly household income exceeds more than $5,000 above the aforementioned guide lines, unfortunately, they do not qualify for a BirchBark Foundation financial assistance grant. Please be sure to visit our resources page to view our list of other financial assistance programs that may be able to help.

If you are qualified and move forward with the application form, you must submit Proof of Care Credit Approval, 3 months of bank statements and 2 additional forms of financial documentation, as well as, pictures of your pet for your application to be considered complete.
